FOUR EASY STEPS TO GETTING YOUR CUSTOM MADE PLAYSAFE MOUTHGUARD
1. FIND YOUR PLAYSAFE DENTIST OR CLINICAL DENTAL TECHNICIAN
Your dentist or clinical dental technician may already be a PLAYSAFE supplier; if not give them our web address or contact details below so that they can make contact.

Costs may vary depending on your dentist’s scale of charges - however, the cost of a mouthguard is very little in comparison to the damage and pain it could prevent.
2. DOWNLOAD THE PRESCRIPTION FORM

Once downloaded please complete the prescription form and take it with you to your dental appointment so that you can give it to your dentist as he will need to complete his section and then send the completed prescription form with your moulds directly to us so that we can then make your custom made Playsafe Mouthguard.
3. CHOOSE YOUR STYLE AND COLOUR
On this website you will find guides to the different levels of protection we offer and also a huge choice of colours, stripes and patterns.

Your dentist will help you choose the level of protection you need and provide you with our full colour chart to choose your colour and pattern.
It is even possible to include artwork such as your school or club logo permanently sealed within the front of your mouthguard, subject to it being copyright free or you having written permission from the owner of the copyrighted artwork. You will need to inform your dentist or clinical dental technician of your requirements and send the artwork directly to us as a.jpg by email, with your dentist or clinical dental technician name, address and contact telephone number, and your name.
Your Playsafe Mouthguards comes with your name permanently sealed within the plastic of each mouthguard as well as an x-ray opaque strip.
4. ENJOY SERIOUS SPORTS PROTECTION!
The process of making your tooth arches is painless and quick. Please allow at least a week for your custom-made mouthguard to be constructed. You can then enjoy ultimate protection for your teeth and gums!
Every Playsafe Mouthguard is supplied with a carrying container and care instructions.
